Home Income Survey (Pre-K - 8th Grade)
Photocopy of Parent/Guardian Identification (Pre-K - 8th Grade) Ex. State ID/Driver’s License, Passport, or Military ID
Proof of California Residency (Pre-K - 8th Grade) Ex. Utility bill, pay stub, rental agreement/property lease or mortgage statement,
Additional Documents (Pre-K - 8th Grade) Ex. Medical Authorization Forms, IEP/SST/504 Plan, Court Orders
NOTE: All information regarding your student’s enrollment will be emailed to the email address provided in their application. All documents must be submitted together, incomplete packets will not be accepted.
